Nurse Navigator

About the role

The University of Miami/UHealth Department of Medicine seeks a full‑time Nurse Navigator to provide proactive clinical navigation for new and complex rheumatology patients. The role involves chart preparation, barrier assessment, patient education, and multidisciplinary coordination across the care continuum.

Key responsibilities

• Prepare clinical charts for new rheumatology patients by reviewing referrals, external records, labs, imaging, medication history and other relevant information.
• Identify missing documentation and direct Patient Navigators to obtain it.
• Assess barriers to care, coordinate resources, and ensure timely access and continuity of treatment plans.
• Serve as a clinical liaison among patients, caregivers, rheumatology providers, primary care, external specialists and other departments.
• Lead the daily workflow of two Patient Navigators, prioritizing tasks and reviewing escalated cases.
• Support quality‑improvement activities related to patient readiness, access and experience.

Required profile

• Bachelor’s degree in a relevant field.
• Valid State of Florida Registered Nurse (RN) license.
• Minimum two years of relevant nursing or patient‑navigation experience.
• Strong interpersonal and communication skills, both oral and written.

Required skills

• Proficiency with Microsoft Office applications.
• Ability to collect, organize and analyze clinical data.
