Nurse Navigator – Cardiac Surgery

About the role

The Nurse Navigator 1 serves as the initial point of contact for patients entering the cardiac surgery pathway, ensuring timely scheduling, coordination of care, and ongoing support throughout the care continuum.

Key responsibilities

• Triages new patient appointments, reviews outside medical records, and schedules initial cardiac surgery consultations.
• Coordinates pre‑clinic assessments, reviews diagnostic testing (echocardiograms, catheterizations, CT, stress tests, labs) and evaluates patient readiness for surgery.
• Acts as liaison among patients, families, multidisciplinary teams, insurance carriers, and external providers to facilitate authorizations and continuity of care.
• Provides patient education on surgical procedures, pre‑operative preparation, hospital stay, and post‑operative recovery, including referrals to cardiac rehabilitation and home health services.
• Supports clinic operations, including patient intake, chart preparation, vital signs, medication reconciliation, and order placement.
• Leads quality improvement initiatives, identifies workflow enhancements, and serves as a clinical mentor and preceptor.

Required profile

• Bachelor’s degree in nursing (BSN) and valid Florida RN license.
• Minimum 2 years of relevant nursing experience, preferably in cardiac or surgical settings.
• Strong communication, teamwork, and learning agility to adapt to evolving protocols.

Required skills

• Proficiency with Epic electronic medical record system.
