Nurse Case Manager - Home Health

About the role

Amedisys, part of the Optum family, offers home health, hospice and community-based care. As a Nurse Case Manager, you will guide patients and families through the care continuum by assessing needs, coordinating care, supporting discharge and transition planning and managing ongoing outpatient care, under the supervision of an RN or MD.

Key responsibilities

• Perform patient assessments and help develop and implement a plan of care
• Make referrals to other disciplines or document why they are not made
• Teach patients self-care techniques and promote health and independence
• Supervise LPNs and HHAs
• Complete documentation timely and accurately and share patient progress with the care team
• Plan and deliver staff education and take on-call assignments
• Take part in clinical development and continuing education

Required profile

• Current RN license for the state of assignment
• At least 1 year of clinical RN experience
• Current CPR certification
• Valid driver's license, reliable transportation and liability insurance

What we offer

• $15,000 sign-on bonus for external candidates
• Total cash compensation of $67,905 to $101,857 per year, plus $39.18 to $58.76 per visit point and an hourly rate, depending on productivity
