New Grad Physical Therapist - Home Health

About the role

BAYADA Home Health Care is hiring a new-graduate Physical Therapist for its Concord home health practice in Lebanon, Vermont. The role helps clients regain independence at home, with a paid training program, mentorship and flexible scheduling. Full-time and part-time opportunities are available.

Key responsibilities

• Perform mobility, strength, balance and safety assessments
• Develop individualized, evidence-based treatment plans
• Provide gait training, manual therapy, balance work and therapeutic exercise
• Implement skin and wound-prevention strategies
• Educate clients and families to promote long-term success
• Document care using the EMR system

Required profile

• Current Vermont PT license (or eligibility)
• Graduation from an accredited PT program
• Comfort with EMR systems (training provided)
