Medication Therapy Management (MTM) Pharmacist

About the role

The MTM Pharmacist provides clinical pharmacy services over the phone, conducting comprehensive and targeted medication reviews, counseling patients on adherence, and ensuring accurate medication reconciliation. This role supports quality initiatives and improves patient outcomes through proactive engagement.

Key responsibilities

• Engage patients telephonically to deliver Medication Therapy Management activities, including CMRs, TMRs, adherence counseling, and education.
• Review clinical interventions and documentation for quality and consistency.
• Maintain accurate patient records, prepare outcome reports, and track program performance.
• Collaborate with patients, providers, pharmacies, and other stakeholders to resolve issues and support transitions of care.
• Develop and implement interventions to optimize medication use and support quality metrics.
• Manage high volumes of interactions while meeting productivity, service, and documentation standards.
• Identify workflow improvement opportunities and contribute to continuous process improvement.
• Provide guidance, onboarding, and training support to team members.

Required profile

• Doctor of Pharmacy (PharmD) from an accredited institution.
• Active Utah pharmacist license in good standing (additional state licenses preferred).
• Knowledge of CMS quality measures, Star Ratings, HEDIS metrics, and Medicare Part C/D medication measures.
• Strong understanding of chronic disease management and complex medication regimens.
• Excellent communication, cultural sensitivity, and ability to handle sensitive conversations.
• Detail‑oriented with a commitment to high‑quality documentation.
• Self‑motivated, able to prioritize tasks, and meet deadlines in a fast‑paced environment.

Required skills

• Proficiency with multiple technology systems in a phone‑based environment.
