LPN Ambulatory – Primary Care for Adults and Pediatrics

About the role

The Cleveland Clinic Health System is seeking a Licensed Practical Nurse (LPN) to join the Ambulatory Primary Care team at the Willoughby Hills Family Health Center. This full‑time position supports both adult and pediatric patients in a fast‑paced outpatient setting, providing direct patient care and collaborating with a multidisciplinary team.

Key responsibilities

• Collect and document objective and subjective patient data, perform nursing assessments, and report findings.
• Implement nursing care plans, administer medications and treatments, and monitor patient responses.
• Observe patients for adverse reactions, perform routine laboratory tests, and educate patients and families.
• Maintain compliance with infection control policies and use personal protective equipment as required.

Required profile

• Graduate of an approved Practical Nursing program.
• Current Ohio LPN license.
• Basic Life Support (BLS) certification through the American Heart Association or American Red Cross.
• One year of patient‑care experience preferred.
