LPN – Adult & Pediatric Home Health Nurse

About the role

BAYADA Home Health Care is seeking a Licensed Practical Nurse to provide adult and pediatric care in clients' homes across Philadelphia. You will deliver high‑quality, one‑to‑one care, including advanced procedures such as tracheostomy and ventilator management, while enjoying flexible scheduling.

Key responsibilities

• Travel to a client’s home on a schedule you choose.
• Execute and document physicians’ orders.
• Perform assessments, monitor conditions, and adjust care plans.
• Document observations, interventions, and evaluations accurately.
• Provide skilled nursing care with autonomy in the home setting.

Required profile

• Current valid nursing license and graduation from an accredited nursing program.
• CPR certification in good standing.
• Availability for 8‑, 10‑ or 12‑hour shifts on 1st, 2nd, or 3rd shift, PRN, part‑time or full‑time.

Required skills

• CPR proficiency.
• Ability to manage tracheostomy and ventilator care.
• Strong documentation and assessment skills.
