Licensed Practical Nurse (LPN) – Home Health Care

About the role

BAYADA Home Health Care is seeking a Licensed Practical Nurse (LPN) passionate about pediatric and adult care to join its Skilled Nursing team in the Laconia area. You will provide one‑to‑one home health services, ensuring high‑quality, compassionate care for clients and their families.

Key responsibilities

• Travel to a client’s home on a schedule you choose and deliver personalized care.
• Follow up on, execute, and accurately document physicians’ orders.
• Perform assessments, monitor client conditions, and record observations.
• Document interventions, evaluations, and any changes in client status.

Required profile

• Graduation from an accredited nursing program (transcript or diploma required).
• Current LPN license in good standing in the state of New Hampshire.
• Current CPR certification.
