Licensed Practical Nurse – Home Health Visits

About the role

BAYADA Home Health Care is seeking a full‑time Licensed Practical Nurse to provide skilled nursing and rehabilitative care during home health visits in the Denver area. You will join a multidisciplinary team delivering compassionate, high‑quality care that enables clients to remain safely at home.

Key responsibilities

• Conduct comprehensive head‑to‑toe assessments and identify subtle changes in patient condition.
• Manage acute and post‑acute care needs, including CHF, COPD, wound care, IV therapy, and post‑surgical care.
• Triage urgent clinical situations independently and make real‑time decisions in the home setting.
• Perform medication reconciliation, patient education, and safety monitoring.
• Document wound assessments, care plans, and point‑of‑care notes accurately.

Required profile

• Current Colorado LPN license in good standing.
• Graduated from an accredited nursing program.
• At least one year of recent hands‑on experience with adult/geriatric patients.
• Ability to work independently while knowing when to escalate concerns.
• Strong problem‑solving skills in unpredictable home environments.

Required skills

• Acute and post‑acute care management (CHF, COPD, wound care, IV therapy).
• Medication reconciliation and patient teaching.
