Licensed Practical Nurse – Full-Time

About the role

ChesPenn Health Services is seeking a patient‑focused Licensed Practical Nurse (LPN) to join its multidisciplinary team at the Chester, PA primary‑care site. The LPN will provide direct clinical support in a busy Federally Qualified Health Center serving diverse, underserved populations.

Key responsibilities

• Perform clinical duties under the direction of a licensed provider or RN, including obtaining histories, vital signs, and preparing patients for visits.
• Supervise and lead a team of staff, ensuring continuity of care and compliance with HRSA, state and federal regulations.
• Administer injections, vaccinations, and conduct venipuncture for laboratory testing.
• Document patient care accurately in the electronic health record and support quality‑improvement initiatives.

Required profile

• Completion of an accredited Practical Nursing program and current, unrestricted Pennsylvania LPN license.
• Current Basic Life Support (BLS) or CPR certification.
• Prior experience in an outpatient or community health setting, preferably with diverse, high‑risk populations.
• Supervisory experience with demonstrated ability to lead teams.

Required skills

• Basic Life Support (BLS) / CPR certification.

• Administration of injections and vaccinations per protocol.
• Proficiency with electronic health record systems.
