Licensed Clinical Social Worker

About the role

Petaluma Health Center, serving Southern Sonoma County and West Marin, is hiring a full-time Licensed Clinical Social Worker to deliver high-quality psychological services in an integrated care setting. The LCSW works as a flexible member of a multidisciplinary healthcare team, assessing and diagnosing mental health and chronic conditions, designing interventions and coordinating care with community resources. The role also includes same-day and crisis services, contributing to clinic quality goals and performing other duties within the federal scope, such as evening shifts and assignments to school-based clinics and homeless shelters.

Key responsibilities

• Work as a collaborative member of the integrated care team, accepting warm handoffs and sharing skills and interventions with colleagues
• Use screening tools to assess mood, risk and progress, and diagnose mental health disorders according to DSM-V criteria
• Determine the appropriate level of treatment, coordinate care with community resources and use outcomes-based practice tools
• Provide psychological assessment and brief treatment to individuals and families, document progress in medical records and collaborate with psychiatrists and other specialists
• Facilitate same-day access for patients needing behavioral health treatment or assessment
• Meet productivity standards set by the medical director and senior management, and take part in quality improvement initiatives
• Maintain current credentials, including the California Social Work license, CE hours, CPR certification and any other required licenses

Required profile

• Master's degree in Social Work
• Current California Social Work license and ability to meet the center's credentialing and privileging requirements
• Bilingual in Spanish
• Ability to read and interpret professional literature and regulations, write reports and communicate clearly with patients, staff and the public
