Internal Medicine Provider

About the role

LaSante Health Center is seeking an Internal Medicine Provider to deliver comprehensive primary and preventive care to adult patients in a culturally diverse, community‑based setting in Brooklyn. The role focuses on managing chronic conditions, promoting wellness, and collaborating with a multidisciplinary team.

Key responsibilities

• Provide primary and preventive care for adult patients, including physical exams, screenings, and chronic disease management.
• Diagnose and treat acute and chronic medical conditions.
• Collaborate with multidisciplinary teams to coordinate patient‑centered care.
• Counsel patients on lifestyle modification, disease prevention, and medication management.
• Maintain accurate, timely documentation in the electronic health record (EHR).
• Participate in quality improvement initiatives and team meetings.
• Complete additional tasks as directed by the supervisor.

Required profile

• Medical degree (MD, DO, NP, or PA) from an accredited institution.
• Board certification or eligibility in Internal Medicine or Adult Primary Care.
• Current New York State license or eligibility for licensure.
• Experience in community health or outpatient primary care preferred.
• Excellent communication and interpersonal skills.
• Bilingual Creole/English strongly preferred.

Required skills

• Proficiency with Electronic Health Record (EHR) systems.
