HouseCalls Nurse Practitioner or Physician Assistant

About the role

Optum HouseCalls, part of the Optum family of businesses, is looking for a Nurse Practitioner or Physician Assistant to join its HouseCalls Advanced Practice Clinician team. The role conducts annual in-home health assessments for Medicare Advantage and other plan members in a non-prescribing, field-based capacity focused on education, gap closure and collaboration with primary care providers. It is a part-time position of 40 hours per week with flexible scheduling between 8am and 8pm over a 7-day week, with no on-call, weekend or holiday requirements.

Key responsibilities

• Conduct comprehensive in-home assessments: past medical history review, medication reconciliation, vital signs and physical exam
• Perform evidence-based screenings and point-of-care testing as appropriate
• Identify and document diagnoses for care management and treatment planning
• Communicate findings to members' primary care providers to address gaps in care
• Recognize urgent or emergent situations and intervene appropriately
• Educate members on disease processes, medications and compliance
• Address social determinants of health and provide referrals as needed

Required profile

• NPs: active ANCC or AANP certification in Family, Adult, Geriatric or Adult-Gerontology Primary Care
• PAs: current NCCPA certification and state licensure
• Active, unrestricted state licensure (or ability to obtain it by the start date) without disciplinary actions or practice limitations
• Active, unrestricted driver's license and reliable transportation to complete home visits
• Ability to navigate varied home environments and to sit, stand or kneel as needed
• Preferred: 1+ years of clinical experience (family, geriatric or home health), proficiency with electronic medical records, ability to carry equipment up to 30 pounds and navigate stairs, proven communication with geriatric or Medicare populations

• Compensation generally ranging from $109,500 to $164,000, based on factors such as local labor market, education and experience
