Hospice Nurse Practitioner (PRN)

About the role

Amedisys, part of the Optum family, provides home health, hospice and community-based care. As a per diem Hospice Nurse Practitioner, you will deliver compassionate, patient-centered care to people with life-limiting illnesses, working with an interdisciplinary hospice team to focus on comfort, dignity and quality of life.

Key responsibilities

• Communicate significant findings, problems, changes in condition or environment to the clinical manager, attending physician and the interdisciplinary team.
• Complete face-to-face encounters and assess disease-related changes, functional decline, adherence to and effectiveness of pain and symptom regimens, patient and family coping and quality of life.
• Order and review labs, x-rays, EKGs and other diagnostics for palliation and treatment, document the treatment plan and review results with the collaborating physician.
• Make referrals to other disciplines as needed and actively take part in interdisciplinary team conferences.

Required profile

• Current, unencumbered Nurse Practitioner license in the state of assignment, with authority to practice and prescribe.
• Medicare billing privileges as a nurse practitioner and state-required malpractice insurance (or group coverage).
• Active collaborating physician agreements meeting state requirements; copies must be supplied.
• 1+ year of experience as a Nurse Practitioner or 1+ year as a hospice Registered Nurse in the last three years.
• Knowledge of Medicare reimbursement and coding across settings, Medicaid and other regulatory requirements.
• Driver's license and reliable transportation.
