Hospice Admissions RN

About the role

Amedisys, part of the Optum family of businesses, offers home health, hospice and community-based services. As a Hospice Admissions RN, you will guide patients and families through the admission process by assessing their physical, psychosocial and spiritual needs and helping establish an individualized plan of care. You will work with physicians, facilities and the interdisciplinary team to coordinate services and provide skilled interventions and education.

Key responsibilities

• Coordinate admissions of hospice patients by assessing their initial physical, psychosocial and spiritual needs
• Coordinate the initial plan of care with the attending physician, medical director and the interdisciplinary team
• Provide skilled interventions and instructions to patients and caregivers for symptom control and comfort under physician direction
• Act as liaison between disciplines and facilities during admission
• Maintain individualized patient care progress records accurately and on time, and document pain and symptom management changes
• Report significant findings, problems and changes in condition to the clinical manager, physician and care team, and obtain physician orders as needed
• Perform on-call duties and other assigned tasks

Required profile

• Current RN license for the state(s) of assignment
• Current CPR certification
• 2+ years of clinical RN experience in acute care settings
• Valid driver's license, reliable transportation and liability insurance
• Preferred: EMR proficiency, ability to work independently and manage multiple priorities, comfort with daily travel between facilities, strong communication and interpersonal skills

• Salary of $71,640 to $107,460 annually for full-time employment, depending on factors such as experience and certifications
