Home Health Registered Nurse (RN)

About the role

Enhabit Home Health & Hospice is seeking a Registered Nurse to provide home health services, coordinating patient care and serving as the primary contact for multidisciplinary teams.

Key responsibilities

• Conduct patient visits and coordinate client care across disciplines.
• Manage visit frequency for each episode of care.
• Consult with physicians and communicate patient updates.
• Accurately collect OASIS data and maintain up‑to‑date medication profiles.
• Report lab values to physicians promptly.
• Participate in weekly case conferences and monthly case manager meetings.

Required profile

• Current RN license in the state of employment.
• At least one year of clinical experience (preferred).
• Demonstrated knowledge of current nursing practice.
• Valid driver’s license and automobile liability insurance.
