Home Health Physical Therapist and Physical Therapist Assistant

About the role

AdvisaCare, a home healthcare provider founded in 1997, is hiring part-time Physical Therapists (PTs) and Physical Therapist Assistants (PTAs) in Macomb, Wayne and Washtenaw counties. The role provides personalized, one-on-one care that helps patients regain strength, improve mobility, reduce pain and return safely to the activities they enjoy.

Key responsibilities

• Conduct comprehensive physical therapy evaluations, develop individualized treatment plans, deliver evidence-based interventions and monitor progress
• Provide skilled physical therapy services according to physician orders and the CMS Conditions of Participation
• Maintain timely, accurate documentation in line with CMS, payer and agency standards
• Collaborate with patients, caregivers, physicians and the interdisciplinary team on mobility, functional outcomes and fall prevention
• Educate patients and caregivers on exercises, gait training, transfers, fall prevention, assistive devices and Home Exercise Programs
• Reassess progress and modify treatment plans, communicating changes to the care team

Required profile

• Degree from an accredited Physical Therapy or Physical Therapist Assistant program
• Current Michigan PT/PTA license in good standing
• Home health experience preferred (at least one year)
• Reliable transportation, valid driver's license and current auto insurance

• Strong clinical judgment, critical thinking and problem-solving
• Excellent communication, interpersonal and patient education skills
• Ability to work independently while collaborating with an interdisciplinary team
