Home Health Occupational Therapist

About the role

BAYADA Home Health Care is seeking an experienced Occupational Therapist to provide per‑diem or part‑time home health visits for adult and geriatric patients throughout Manassas, VA. You will join a multidisciplinary team delivering rehabilitative care that enables clients to remain safely at home.

Key responsibilities

• Conduct home visits in designated geographic territories and perform comprehensive functional evaluations.
• Develop individualized intervention plans, assess home environments for hazards, and recommend equipment or adaptations.
• Collaborate with nursing staff on multidisciplinary care plans and educate patients and families on therapeutic activities.
• Document all observations, interventions, and evaluations using a tablet‑based electronic system.

Required profile

• Current Virginia Occupational Therapist license.
• Minimum one year of recent OT experience; prior home‑care experience preferred.
• Graduation from an accredited OT program.
• Ability to work independently, strong organizational and problem‑solving skills.

Required skills

• Proficiency with electronic medical records (EMR) systems.
• Solid computer skills for documentation on tablet devices.
