About the role

Summit Home Health Care is seeking compassionate Home Health Aides (HHA) or Personal Care Assistants (PCA) to provide in‑home support to patients in the New York area. The role involves assisting clients with daily living activities, ensuring their comfort and safety while following individualized care plans.

Key responsibilities

• Assist patients with personal hygiene tasks such as bathing, oral care, dressing, and grooming.
• Support routine activities outlined in the patient’s care plan, including mobility and basic exercises.
• Monitor patients’ condition and report any changes to supervisory staff.
• Prepare light meals or snacks and help with feeding when needed.
• Accompany patients to medical appointments, grocery shopping, and other essential errands.

Required profile

• Valid HHA or PCA certification.
• State ID, Social Security card, and work authorization if required.
• Two professional reference letters.
• Current physical examination, Quantiferon test, Rubella and MMR lab reports, and a drug screen completed within the last six months.

Required skills

• Ability to follow a prescribed care plan.
• Basic knowledge of hygiene and nutrition support.
• Effective communication with patients and healthcare team.
