About the role

Key responsibilities

• Conduct home visits to assess patients' functional status, including muscle function, endurance, coordination, communication, ADL and IADL performance.
• Develop and continually revise individualized intervention plans, evaluate home environments for hazards, and recommend equipment or adaptations.
• Document observations, interventions, and evaluations using a touch‑pad tablet and collaborate with the interdisciplinary care team.

Required profile

• Current Missouri Occupational Therapist license.
• At least one year of recent OT experience (home care experience preferred but not required).
• Graduated from an accredited OT program and able to work independently.

Required skills

• Proficiency with electronic medical records (EMR) systems.
