Field Nurse Practitioner (PRN, 1099) - In Home Assessments

About the role

Advantmed is seeking a licensed, dependable Nurse Practitioner to conduct structured, in-home wellness and risk-adjustment assessments for patients with varying healthcare needs. This is a 1099 independent contractor (PRN) role offering flexible weekday availability, no evenings, weekends or on-call duty, and steady supplemental income rather than short-term or ad hoc work.

Key responsibilities

• Conduct in-home annual wellness visits and chronic condition assessments
• Take focused histories, perform exams and vitals, review medications and run screenings such as hemoglobin A1C, spirometry, KED, DRE and FITE
• Educate members on preventive care and chronic disease management
• Document visits in the EMR to support value-based care and risk adjustment
• Maintain ongoing monthly availability, respond during onboarding and scheduling, and honor confirmed visits

Required profile

• Active, unencumbered NP license in the applicable state
• 3+ years of patient care experience preferred
• Preferred: in-home or risk adjustment experience
• Willingness to obtain additional licensure if needed (supported by Advantmed)
• Minimum commitment of about 30 hours per month, ongoing

Required skills

• Comfortable with EMR systems and able to work independently in the field
• Daily travel within a 55-mile radius of the assigned county
