Field Nurse Practitioner - In-Home Assessments (Sumter County, FL)

About the role

Advantmed is seeking a licensed, dependable Nurse Practitioner to conduct structured, in-home wellness and risk-adjustment assessments for patients with varying healthcare needs. The role is a 1099 PRN position offering flexible weekday availability, no evenings, weekends or on-call duty, and steady supplemental income. Providers earn about $100 per completed assessment plus paid mileage.

Key responsibilities

• Conduct in-home Annual Wellness Visits and chronic condition assessments
• Take focused histories, perform exams and vitals, review medications and run screenings such as hemoglobin A1C, spirometry, KED, DRE and FITE
• Educate members on preventive care and chronic disease management
• Document visits in the EMR to support value-based care and risk adjustment
• Maintain ongoing monthly availability, honor confirmed visits and work independently in the field

Required profile

• Active, unencumbered NP license in the state where you will work
• 3+ years of patient care experience preferred
• Willingness to travel throughout Lake, Sumter and Citrus counties, Florida, within a 55-mile radius of the assigned home, with reliable transportation
• Preferred: in-home or risk-adjustment experience, weekday availability, standard hours, previous 1099 experience, bilingualism
• Minimum commitment of about 30 hours per month, ongoing
