Field Nurse Practitioner - In-Home Assessments

About the role

Advantmed is seeking a licensed, dependable Nurse Practitioner to conduct structured, in-home wellness and risk-adjustment assessments for patients with varying healthcare needs. The role is a 1099 PRN position offering flexible weekday availability, no evenings, weekends or on-call duty, and steady supplemental income. Pay is about $100 per completed assessment plus paid mileage.

Key responsibilities

• Conduct in-home annual wellness visits and chronic condition assessments
• Take focused histories, perform exams and vitals, review medications and run screenings such as hemoglobin A1C, spirometry, KED, DRE and FITE
• Educate members on preventive care and chronic disease management
• Document visits in the EMR to support value-based care and risk adjustment
• Maintain ongoing monthly availability and honor confirmed visits and assignments

Required profile

• Active, unencumbered NP license in the state where you will work
• 3+ years of patient care experience preferred
• Previous in-home or risk-adjustment experience preferred
• Willingness to obtain additional licensure if needed, supported by Advantmed
• Reliable transportation to reach members' homes within the assigned area
• Primary coverage in Plummer, ID, with occasional visits in Pinehurst, ID

Required skills
