Family Nurse Practitioner – Enhanced Primary Care

About the role

The Article 28 Primary Care Clinic is expanding and seeks a compassionate Family Nurse Practitioner to join its interdisciplinary team. This position focuses on delivering high‑quality, person‑centered primary care to individuals with intellectual and developmental disabilities (IDD) across the lifespan.

Key responsibilities

• Provide comprehensive primary and preventive care within an Article 28 framework.
• Conduct assessments, diagnose conditions, and develop evidence‑based treatment plans.
• Collaborate with physicians, providers, and interdisciplinary teams to coordinate individualized care.
• Support health promotion, patient education, and chronic disease management tailored to the IDD population.
• Participate in quality‑improvement initiatives and help refine clinic workflows and protocols.
• Offer clinical leadership and mentorship to nursing and support staff.
• Maintain accurate clinical documentation in line with regulatory standards.
• Deliver services both in‑person and via telehealth as appropriate.

Required profile

• Current New York State Family Nurse Practitioner license and certification (MSN or DNP required).
• Minimum of 3 years of clinical experience in primary care, family medicine, or internal medicine.
• Commitment to inclusive, equitable, person‑centered care for individuals with IDD.
• Willingness to obtain IDD‑specific certifications and complete related training.
• Professional verbal and written communication skills.
• Ability to meet vehicle‑driving requirements for Springbrook vehicles.
