Data Entry Clerk – Enhanced Care Management (Onsite, Los Angeles)

About the role

The Data Entry Clerk for the Enhanced Care Management (ECM) – Community Supports program will support Amity Foundation’s prison and re‑entry initiatives by accurately entering and maintaining participant information in the electronic health record system. This onsite position in Los Angeles contributes to a patient‑centered approach that improves outcomes for high‑need populations.

Key responsibilities

• Enter new ECM student/member demographics, referral details, admission dates, and required program fields into the EHR system with a high level of accuracy.
• Upload all required documentation (consents, eligibility verification, ROI, care plans, assessments, outreach logs) into the correct EHR categories.
• Maintain naming conventions and filing standards to ensure documents are easily accessible for audits and quality checks.
• Monitor incoming referrals and promptly initiate EHR entries in alignment with MCP timelines.
• Prepare reports from the EHR for timely claim submission and coordinate with the accounting department for proper reimbursement.
• Verify that admission and enrollment data meet Medi‑Cal, DHCS, and individual MCP requirements.
• Track missing documents, follow up with Case Managers, and ensure complete client charts for billing readiness.
• Support QA audits by uploading documentation ahead of internal and MCP audits.

Required profile

• Strong attention to detail and commitment to data accuracy.
• Ability to work collaboratively with Case Managers, LVNs, CHWs, BHCMs, and QA staff.
• Familiarity with healthcare regulations such as Medi‑Cal and DHCS is advantageous.
