Consultative Pharmacist – $5,000 Sign‑On Bonus

About the role

We are seeking a licensed pharmacist to join our consultative team supporting hospice and palliative‑care patients. This position offers a $5,000 sign‑on bonus and the chance to improve medication safety and quality of life for individuals at the end of life.

Key responsibilities

• Provide expert guidance on pain and symptom management tailored to each patient.
• Collaborate with nurses, hospice staff, and caregivers to develop cost‑conscious medication care plans.
• Educate healthcare professionals and caregivers on proper medication use and adverse‑event prevention.
• Document clinical interventions, patient outcomes, and medication profiles in electronic health systems.
• Maintain compliance with HIPAA and other regulatory requirements.
• Participate in rotating weekend coverage and support a fast‑paced call‑center environment.

Required profile

• Active pharmacy license in Pennsylvania (or willingness to obtain one).
• PharmD or B.S. in Pharmacy from an ACPE‑accredited program.
• Experience in hospice, palliative‑care, or oncology settings preferred.
• Previous experience in a remote clinical consultation or call‑center setting is a plus.

Required skills

• Proficiency with Electronic Health Record (EHR) systems.
• Knowledge of HIPAA privacy and security standards.
• Familiarity with pharmacy benefit management processes.
