Community Outreach Representative

About the role

Alpine Physicians is hiring a Community Outreach Representative in Lakewood to engage Medicare seniors and create organic patient growth in local communities. The representative works closely with local community partners and clinics so seniors can access value-based preventive primary care, reports to the local Outreach Community Manager and works 50% field and 50% home office.

Key responsibilities

• Deliver and coordinate educational presentations to existing and prospective patients.
• Create and maintain relationships with referral sources and develop and maintain local broker relationships.
• Attend lead-generating events in the community and at clinics, and convert prospective patients to enrolled, active patients with booked visits and an assigned PCP.
• Engage potential members by phone, text and email, and manage and report leads through CRM or other tools.
• Collaborate with marketing on event promotion, social media and content development, and with operations on prospective patient tours and service coordination.
• Work with non-profits and other community partners to acquire new patients, and with brokers and the local health plan community to raise awareness and increase enrollment.

Required profile

• Bachelor's degree in business, sales, marketing or equivalent work experience.
• At least two years of experience in the healthcare sector.
• At least one year with Medicare; experience with value-based care is a plus.
• Experience in a quota-based compensation model.

• Reporting and tracking with Excel.
• Excellent written and verbal communication skills.

What we offer

• Base salary of $60,000-$65,000 plus incentive compensation, with total earnings up to $120,000 based on performance.
