About the role
Community Care Specialists join a mobile multidisciplinary team that delivers patient‑centered care to adults with co‑occurring mental health and substance‑use disorders. The role focuses on high‑needs, high‑risk clients, supporting their transition from residential treatment to stable community living.
Key responsibilities
- Conduct ongoing client evaluations, update treatment plans, and manage discharges.
- Provide direct and indirect client interaction across in‑person, phone, and digital platforms.
- Coordinate case management services with internal departments and external agencies.
- Assist clients in building life skills, securing employment, and obtaining supportive housing.
- Attend mandatory in‑person staffing, clinical oversight, and Adult Recovery Treatment meetings.
- Complete risk screenings, identify client needs, and process enrollment.
- Meet monthly billable service targets while traveling to various Crossroads facilities and community locations.
Required profile
- High school diploma or GED (minimum).
- At least one year of direct client care experience in substance‑abuse or mental‑health settings.
- Current Arizona Peer Recovery Support Specialist certification preferred.
- Minimum six months of continuous sobriety.
- Valid Arizona driver’s license and clean motor‑vehicle record.
Required skills
- Strong verbal and written communication abilities.
- Capability to work independently and collaboratively within a team.
