Clinical Team Manager RN - Hospice

About the role

The Care Team, in partnership with Traditions Health, is looking for a Clinical Team Manager (Registered Nurse) to join its growing hospice team in Crown Point, Indiana. The role supports branch leadership in educating and training clinicians so they understand documentation requirements that meet regulatory standards, and helps coordinate the relationship between physicians, referral sources, patients, caregivers and staff.

Key responsibilities

• Ensure hospice services are delivered to acceptable standards and company procedures, and review and approve patient information submitted by licensed professionals.
• Assist with patient care review meetings (case conferences and interdisciplinary team meetings), review and approve orders, and oversee patient care.
• Help the branch director with day-to-day office and staff management, and train clinicians on documentation requirements.
• Review on-call coordination notes daily, and back up the intake coordinator and Patient Service Coordinator with referrals, scheduling and missed visits.
• Provide orientation and in-service training to field and office staff, and assist with surveys, staff meetings and community education.
• Perform chart audits, quality assessments and performance improvement activities, and complete onsite supervisory visits.
• May step in as branch leader when the director is away, handling evaluations, counseling and hiring/termination recommendations.
• May act as a licensed professional, including reviewing and approving orders, lab reports, wound scores and vital sign alerts, and may take part in on-call rotation.

Required profile

• Previous experience in home care; experience in Home Care Home Base is preferred.
• Organized, detail-oriented, able to manage several projects at once and work independently with minimal supervision.
• Effective oral and written communication, self-starter attitude.

Required skills

• Advance computer skills preferred.
• Ability to perform duties of a licensed professional as needed.
