Registered Nurse - Home Health

About the role

This Registered Nurse position provides skilled nursing care to clients of all ages in their own homes. The nurse coordinates care with the interdisciplinary team, patients and their families and the referring agency, taking responsibility for care coordination.

Key responsibilities

• Complete initial and ongoing assessments of physical, psychosocial and environmental needs, including OASIS or other assessments, and regularly re-evaluate nursing needs and care outcomes
• Develop, initiate and revise the plan of care, provide preventive and rehabilitative nursing procedures and refer to other services as needed
• Plan for patient discharge and counsel patients and families
• Use infection control measures that protect staff and patients (OSHA)
• Coordinate services, inform physicians and others of changes in condition, and monitor cases for third-party payer compliance
• Prepare clinical and progress notes and participate in in-service programs
• Supervise, teach and direct other nursing personnel, assign home health aides and supervise LPNs/LVNs and paraprofessionals; aide competency evaluations require two years of clinical and one year of home health experience
• Perform on-call duties and promote the agency's policies

Required profile

• Diploma, Associate or Baccalaureate degree from an accredited school of nursing
• Current state Registered Nurse license and current driver's license
• One year of RN experience in a clinical care setting; home health experience preferred
• Reliable transportation, valid driver's license and auto insurance
• Able to work a flexible schedule, travel locally, and cope with variable weather and exposure to blood, bodily fluids and infectious diseases
• Ability to stand and walk for long periods, lift up to 50 lbs and move patients, and make quick decisions under stress
