Clinical Support Team Provider (NP/PA)

About the role

Petaluma Health Center serves Southern Sonoma County and West Marin with patient-centered primary care. The Clinical Support Team Provider is an integral member of the primary care team, offering treatment plan follow-up and support between visits, especially for high-risk or complex patients. The role is primarily remote with 1-2 days per week in person at the Petaluma Clinic, and candidates must live in California.

Key responsibilities

• Support teams by reviewing labs, diagnostic imaging and documents between visits, and share results and follow-up needs with the Clinical Support Team
• Act as backup for urgent questions, result reviews and medication reviews for primary care teams at all sites
• Provide telehealth visits to review urgent diagnostic results and regular in-person or telehealth urgent care
• Help patients set health goals, schedule appointments and understand their care
• Order diagnostics and implement therapies within scope of practice, and take part in Medical Staff meetings and committees
• Maintain credentials, including California Medical License, DEA license, CME hours and BLS certification

Required profile

• Nationally accredited Nurse Practitioner or Physician Assistant program; two years of primary care experience or a one-year NP residency
• Current California NP or PA license and national board certification by day one
• Active and/or transferable DEA license by day 90; RN and NP/Furnishing licenses for NPs
• Preferred: experience in a NCQA Patient-Centered Medical Home and a Joint Commission-accredited organization
• Bilingual English and Spanish highly preferred
