Clinical Pharmacist – Rheumatology Ambulatory

About the role

The Clinical Pharmacist in the Rheumatology Ambulatory setting is responsible for comprehensive medication management of assigned patients. This role involves evaluating therapy appropriateness, providing education, and supporting pharmacy and medical residents.

Key responsibilities

• Conduct pharmacokinetic consultations, using laboratory data to select and dose medications.
• Perform medication reconciliation, monitoring, and patient education to ensure safe therapy.
• Develop and review order sets, formularies, and clinical pathways for the rheumatology population.
• Mentor residents, students, and new staff, and contribute to departmental education initiatives.

Required profile

• Bachelor of Science in Pharmacy; PharmD preferred.
• Completion of a PGY1 Pharmacy Practice Residency (or 3 years related specialty experience); PGY2 residency preferred.
• Current state pharmacy license.
• Effective verbal and written communication skills.

Required skills

• Pharmacokinetic consultation and medication therapy management.
• Medication reconciliation and patient education.
• Proficiency with Windows‑style computer applications and data entry.
