Clinical Pharmacist

About the role

Clearway Health is seeking a Clinical Pharmacist to ensure optimal patient outcomes for chronic and specialty drug‑related disease states. You will serve as a medication expert for patients, families, clinicians and will help educate pharmacy trainees.

Key responsibilities

• Conduct comprehensive medication therapy reviews for patients on specialty medications, ensuring safety, efficacy, and adherence.
• Provide disease‑specific counseling, support prior authorizations, appeals and benefits verification.
• Monitor clinical outcomes, adverse events and adherence metrics, documenting interventions for quality improvement.
• Ensure specialty pharmacy activities comply with URAC/ACHC standards, manufacturer agreements and payor requirements.

Required profile

• PharmD or bachelor's degree from an accredited College of Pharmacy.
• PGY‑1 residency or 3+ years of direct patient‑care clinical pharmacy experience.
• Active pharmacist licensure (or ability to obtain) in the state of employment.
• Preceptor license obtainable within one year of hire.

Required skills

• Expertise in medication therapy management, drug interactions and disease states.
• Proficiency with Microsoft Office (Word, Excel, Outlook) and web browsers.
• Strong documentation and reporting abilities for audits and accreditation.
• Ability to manage multiple tasks, prioritize duties and meet deadlines.

What we offer

• Annual base salary range $133,000 – $166,000.
• Eligibility for company bonus program and comprehensive benefits package.
