Clinical Pharmacist

About the role

We are seeking an experienced Clinical Pharmacist to join our patient‑centered healthcare organization. The role focuses on delivering safe, evidence‑based medication therapy across acute‑care and, when assigned, ambulatory settings. You will work closely with providers and nurses to improve patient outcomes through expert pharmacotherapy interventions.

Key responsibilities

• Provide pharmacokinetics dosing support and renal dosing protocols.
• Manage anticoagulation protocols to ensure safety and efficacy.
• Conduct medication reconciliation during transitions of care.
• Review drug interactions and allergies to prevent adverse events.
• Support antimicrobial stewardship initiatives.
• Deliver clinical pharmacy services, including patient counseling and medication therapy management.
• Prepare sterile compounds according to established guidelines.
• Verify medication orders for accuracy and intervene when necessary.
• Participate in multidisciplinary committees and develop medication‑use policies.
• Stay current with advancements in pharmacotherapy and apply new knowledge.

Required profile

• Experienced Clinical Pharmacist with a deep understanding of pharmacotherapy.
• Ability to collaborate effectively with multidisciplinary healthcare teams.
