Clinical Pharmacist

About the role

We are seeking an experienced Clinical Pharmacist to join our patient‑centered healthcare organization. This permanent position offers the chance to apply your clinical judgement in a collaborative hospital pharmacy environment, supporting safe and evidence‑based medication use across acute and ambulatory care.

Key responsibilities

• Provide pharmacokinetics dosing support and renal dosing protocols.
• Manage anticoagulation protocols to ensure patient safety.
• Conduct medication reconciliation during transitions of care.
• Review drug interactions and allergies to prevent adverse events.
• Support antimicrobial stewardship initiatives.
• Deliver patient counseling and medication therapy management.
• Prepare sterile compounds following established guidelines.
• Verify medication orders for accuracy and intervene when needed.
• Participate in multidisciplinary committees and develop medication use policies.

Required profile

• Licensed pharmacist with a strong understanding of pharmacotherapy.
• Experience in hospital or acute‑care pharmacy settings.
• Ability to work collaboratively with providers, nurses, and other healthcare professionals.
• Commitment to continuous education and staying current with clinical advances.
