About the role

We are seeking a dedicated Clinical Pharmacist to join our patient‑centered healthcare organization. This permanent position offers the chance to apply advanced pharmacotherapy knowledge in acute‑care and ambulatory settings while collaborating with physicians, nurses, and multidisciplinary teams.

Key responsibilities

• Provide pharmacokinetics and renal dosing support to optimize medication therapy.
• Manage anticoagulation protocols and ensure patient safety.
• Conduct medication reconciliation during transitions of care.
• Review drug interactions and allergies to prevent adverse events.
• Support antimicrobial stewardship initiatives.
• Deliver patient counseling and medication therapy management.
• Prepare sterile compounds following established guidelines.
• Verify medication orders for accuracy and intervene when necessary.
• Participate in multidisciplinary committees to develop medication use policies.

Required profile

• Licensed pharmacist with experience in clinical pharmacy.
• Strong understanding of pharmacokinetics, renal dosing, and anticoagulation.
• Proven ability to perform medication reconciliation and stewardship activities.
• Excellent communication skills and collaborative mindset.
