Clinical Pharmacist (Home Infusion)

About the role

Option Care Health is seeking a licensed Clinical Pharmacist to provide safe and appropriate pharmacy services for patients receiving home infusion therapy. This entry‑level position involves designing and monitoring individualized pharmacy care plans, coordinating with nursing staff, and educating patients and caregivers.

Key responsibilities

• Assess patient eligibility for home infusion care based on medical condition and medication regimen.
• Collect and organize patient‑specific information to identify medication‑related problems.
• Develop, recommend, and monitor individualized pharmacy care plans for home care patients.
• Adjust medication therapy or monitoring parameters as needed to achieve defined outcome goals.
• Perform nutritional assessments and provide related medication recommendations.
• Coordinate patient services with the Nursing Department and other healthcare professionals.
• Educate patients, caregivers, and staff on appropriate medication use and safety.
• Document patient progress and modify care plans based on ongoing evaluation.

Required profile

• Active pharmacist license in the United States.
• Entry‑level clinical pharmacy experience or strong interest in home care pharmacy.
• Ability to assess patient suitability for home infusion services.
• Strong communication skills for patient and interdisciplinary team education.
• Detail‑oriented with ability to collect and analyze patient data.
