Clinical Administrative Coordinator

About the role

UnitedHealth Group (Optum) is hiring a Clinical Administrative Coordinator in Albuquerque, New Mexico. The role acts as the front line for care transitions of vulnerable members, reaching out to new high-risk members, scheduling visits and arranging transportation and supplies so nurses and social workers can focus on clinical and psychosocial work. The position is remote or office-based, with one day onsite in Tempe, Arizona, and travel of about 10% or less.

Key responsibilities

• First outreach to newly identified high-risk members, explaining the program and booking the first RN or social worker visit
• Schedule post-discharge visits before the member leaves the hospital and confirm they happen within 7 days
• Book primary care physician, specialist and lab appointments, and arrange transportation, DME delivery and home health starts
• Track referrals to closure and flag stalled ones to the RN or social worker
• Collect documents for Medicaid/LIS applications and D-SNP recertification under the social worker's direction
• Keep contact information and outreach attempts current in the care management platform

Required profile

• High school diploma or GED
• 2+ years of administrative support experience in a healthcare, medical office or health insurance setting
• Preferred: scheduling experience in healthcare, Medicare Advantage experience, care coordination or community health worker experience

Required skills

• Strong phone skills
• Bilingual Spanish/English preferred
