Case Manager – PRN (Per Diem)

About the role

The Case Manager – PRN (Per Diem) coordinates interdisciplinary care plans for patients from admission through discharge at the University of Miami. This position acts as a liaison among patients, families, and healthcare teams to ensure timely and effective services.

Key responsibilities

• Integrate social services into patient care plans.
• Educate patients about their conditions and treatment options.
• Advocate for patients with external agencies and service providers.
• Design comprehensive health plans covering medication administration, consultations, and rehabilitation.
• Collaborate with multidisciplinary professionals to assess medical and physical needs.
• Identify environmental barriers to patient progress through interviews and record reviews.
• Monitor, evaluate, and document patient progress against measurable goals.
• Utilize social work expertise to coordinate care and ensure service efficacy.
• Follow university and unit policies, safeguarding institutional assets.

Required profile

• Bachelor’s degree in a relevant field.
• Minimum of 3 years of relevant experience.
• Applicable certifications as defined by the department.
• Strong learning agility, teamwork, and communication abilities.

Required skills
