Case Manager

About the role

The Case Manager supports the Clinical Coordinator in delivering comprehensive client services, including treatment planning, case load management, individual and group interventions, crisis response, care coordination, and discharge planning within an outpatient substance‑use disorder program.

Key responsibilities

• Develops and documents individualized treatment plans and monitors client progress.
• Maintains a regulated caseload, completing progress notes, evaluations, and discharge planning in compliance with policies.
• Provides individual and group interventions, including crisis management as needed.
• Coordinates care by reviewing cases, updating weekly progress, and arranging after‑care services.
• Participates in clinical staff meetings, trainings, and other required activities.
• Assists clients with applications for government assistance, court obligations, and referrals to external agencies.

Required profile

• Meets Arizona Department of Health Services personnel requirements (age 21+, appropriate licensure or certification).
• Holds at least a high school diploma or GED with 1–2 years related experience.
• Maintains high ethical standards, sound judgment, and strong organizational abilities.
• Holds current CPR/First Aid/Narcan certification, a negative TB test, and a valid fingerprint clearance card.
• Passes background and Adult Protective Services checks; clean motor vehicle record preferred.
• Demonstrates knowledge of addiction, recovery, and twelve‑step programs.

Required skills

• Proficient with computer applications, Microsoft Office suite, and electronic medical record (EMR) systems.
• Accurate typing skills for timely documentation.
