Case Manager II – Substance Use Disorder Treatment

About the role

Indiana Treatment Centers – Mishawaka is seeking a compassionate, organized Case Manager II to support individuals with substance use disorders. The role focuses on helping patients navigate treatment, overcome barriers, and transition to life beyond the program.

Key responsibilities

• Conduct biopsychosocial assessments and contribute to treatment plan development.
• Coordinate care among clinical, medical, and recovery support teams.
• Provide case management for patients with moderate to high needs, including crisis coordination.
• Facilitate discharge planning and prepare patients for the next level of care.
• Maintain collaborative relationships with community agencies, courts, and referral sources.
• Assist patients in accessing housing, employment, transportation, insurance, and other resources.

Required profile

• Demonstrated compassion and commitment to recovery‑focused care.
• Experience working with individuals facing behavioral health and substance use challenges.
• Strong organizational skills and ability to coordinate multidisciplinary services.
• Excellent communication skills for interacting with patients, families, and community partners.
