Clinical Team Manager (RN) - Home Health & Hospice

About the role

The Care Team, in partnership with Traditions Health, is looking for a Clinical Team Manager (Registered Nurse/RN) in Mishawaka. The manager ensures that home health and/or hospice services are coordinated according to acceptable standards of practice and company procedures, and oversees patient care, scheduling, orders and staff.

Key responsibilities

• Review and approve patient information, orders and care assessment coordination notes submitted by licensed professionals, and approve or decline orders as appropriate
• Assist with patient review meetings (case conferences and interdisciplinary team meetings) and help the branch director with day-to-day office and staff management related to patient care
• Help train branch clinicians on documentation requirements and regulatory standards, and orient and train field and office staff
• Support the intake coordinator and patient service coordinator with referrals, rescheduling and missed visits, and review and approve patient schedules
• Follow up on orders, lab reports, wound score deviations and vital sign alerts, and maintain chart audits and quality improvement activities
• Provide education to patients and families, assist with surveys, attend staff meetings and community education, and act as branch leader when the director is away
• Perform on-call duties and other duties assigned by the supervisor

Required profile

• Previous experience in home care; experience in Home Care Home Base is preferred
• Organized, detail oriented, able to manage several projects at once and work independently with minimal supervision
• Strong communication skills, professional ethics and a service-oriented attitude in a high-stress environment
