Care Navigator – Part‑Time (Florida)

About the role

Join Nolia Health’s transdisciplinary team as a Care Navigator, providing virtual, telehealth‑based support to patients and families dealing with chronic and serious age‑related conditions. You will work remotely, collaborating with clinicians to deliver personalized, culturally responsive care planning and resource navigation.

Key responsibilities

• Serve as the primary point of contact for caregivers and families, conducting biopsychosocial assessments and developing individualized care plans.
• Provide disease‑stage education, connect families to community resources, and coordinate with primary care physicians and specialists.
• Deliver caregiver education on burnout prevention, self‑care, and resilience, and track contact frequency per program policy.
• Document all interactions accurately and represent Nolia Health at community events to generate referrals.

Required profile

• At least 2 years of experience in a healthcare or community‑based setting.
• Understanding of caregiver challenges and cultural humility, with ability to interact respectfully with diverse populations.
• Strong communication skills for handling emotionally charged interactions.

Required skills

• Experience with Electronic Health Records (EHR) systems.
• Proficiency in general computer use, troubleshooting minor technology issues, and setting up remote work equipment.
