About the role

HomeWell Care Services of Northwest Indiana is seeking a dedicated Care Manager to serve as the primary liaison among office staff, caregivers, clients, and families. The role blends administrative coordination, recruitment, and client interaction to ensure high‑quality home care delivery.

Key responsibilities

• Recruit, interview, and onboard care staff candidates.
• Lead caregiver orientation and update program materials as needed.
• Maintain detailed reports, perform data entry, and complete client assessments.
• Develop and implement written care plans for each client.
• Assign and schedule caregivers, manage call‑offs, and ensure continuous client coverage.
• Communicate regularly with clients, families, and designated representatives.
• Supervise caregivers and conduct in‑home supervisory visits.
• Represent the agency at health fairs and job fairs.
• Perform additional duties assigned by management.

Required profile

• 1‑2 years of office experience; home‑care or health‑care background preferred.
• Knowledge of medical terminology, marketing/sales, basic recruiting, or scheduling is a plus.
• Reliable transportation, current insurance, and an unrestricted driver’s license.
• Ability to pass a national/expanded criminal background check.
• Strong organizational skills, attention to detail, and the ability to multitask.
