Care Manager (CNA/CHHA) - Part-Time & Per Diem

About the role

Brandywine by Monarch Communities is hiring a Care Manager (CNA/CHHA) to support residents in daily living needs. The caregiver reports to the Director of Wellness and works part-time or per diem. The salary range is $21.00 per hour.

Key responsibilities

• Assist residents in all aspects of their daily life according to their care plan, including personal care, food service, housekeeping, laundry, behavior management, socialization and activities.
• Provide resident care such as bathing, dressing, toileting and feeding as outlined in the care plan.
• Maintain the safety, comfort, dignity and confidentiality of all residents.
• Escort residents to meals and activities.
• Ensure assigned residents are clean, neatly dressed and groomed, safe and fed during their shifts.
• Document all services provided to assigned residents during each shift.
• Report any change of condition in a resident immediately to the supervisor.

Required profile

• Certified Nursing Assistant or Certified Home Health Aide.
• Knowledge and experience working with the elderly population, preferred.
• Dementia experience preferred.
• Able to stand, walk, sit, use fine and gross motor skills, balance, stoop, kneel and crouch, and to lift or carry up to 50 lbs with proper body mechanics.
