Cardiology Patient Navigator – Remote

About the role

Our client, a busy U.S.-based cardiology team, is seeking a remote Cardiology Patient Navigator to support patient communication, scheduling, and care coordination. The role focuses on ensuring patients follow their care plans, managing high‑volume caseloads, and maintaining accurate clinical documentation.

Key responsibilities

• Manage patient scheduling, reminders, and follow‑up workflows within EMR/EHR systems.
• Conduct telephonic outreach to reinforce care instructions, coordinate next steps, and support patients navigating cardiology pathways, including CHF management.
• Track testing schedules, appointments, and remote patient monitoring workflows for a caseload of 150+ patients.
• Coordinate with the clinical team to ensure continuity of care and escalate clinical concerns to providers when needed.
• Maintain timely, HIPAA‑compliant documentation and support billing and insurance coordination.

Required profile

• Clinical healthcare background (CNA, MA, RN, or equivalent).
• Minimum 3 years of experience in healthcare, telehealth, or medical office settings.
• Experience in care coordination or management, preferably in remote or telephonic environments.
• Bilingual proficiency (B2+ English and B2+ Spanish) is required.
• Strong patient‑centered communication and ability to work independently in a remote setting.
