Ambulatory Care Pharmacist – GI Specialty

About the role

Shields Health Solutions is seeking an Ambulatory Care Pharmacist to join its clinical pharmacy team in Kalamazoo, MI. The role focuses on providing hands‑on, patient‑centered medication therapy management for gastro‑enterology (GI) disease states, building strong relationships with patients and prescribers, and ensuring safe, effective, and cost‑effective drug therapy.

Key responsibilities

• Perform comprehensive medication therapy management, including reconciliation, dose optimization, and adherence counseling for GI patients.
• Collaborate with an interdisciplinary care team to develop individualized care plans aligned with evidence‑based GI guidelines.
• Monitor medication regimens for therapeutic goals, adverse effects, drug interactions, and cost‑saving opportunities.
• Assess and manage infusion and injectable specialty therapies, including biologics used in inflammatory bowel disease.
• Identify, resolve, and prevent medication‑related problems and provide education on injection techniques.

Required profile

• Licensed pharmacist with experience in ambulatory or clinical pharmacy settings.
• Demonstrated expertise in gastro‑enterology or related disease‑state management.
• Strong communication skills to engage patients, prescribers, and the care team.
• Ability to work independently while integrating into a multidisciplinary clinic.
