Advanced Registered Nurse Practitioner (ARNP) – Idaho

About the role

The company is seeking a part‑time Advanced Registered Nurse Practitioner to deliver virtual, telehealth care for senior patients with chronic conditions, beginning with dementia, and to support their family caregivers.

Key responsibilities

• Provide high‑quality telehealth services, develop and adjust personalized care plans, and monitor patient progress.
• Collaborate with a transdisciplinary team—including MDs, neuropsychologists, social workers, and care navigators—to coordinate comprehensive care.
• Supervise care navigators, review and maintain patient charts, and ensure compliant clinical documentation.
• Complete accurate billing for Chronic Care Management (CCM) and other fee‑for‑service programs.

Required profile

• Minimum two years of outpatient experience in independent practice or a larger medical center.
• Current, unrestricted ARNP license in Idaho (additional state licenses preferred).
• Master’s level ARNP education; board certification in geriatric care preferred.
• Experience with chronic, longitudinal care models in digital health.

Required skills

• Telehealth delivery and virtual patient engagement.

• Use of digital health platforms for longitudinal care.
