Admissions Registered Nurse – Tues‑Fri 12pm‑10pm

About the role

The Admissions Registered Nurse joins Ascend Hospice to lead patient admissions during the Tuesday‑Friday shift from noon to 10 pm. This role focuses on assessing new patients and families, creating individualized care plans, and ensuring a smooth transition into hospice services.

Key responsibilities

• Conduct an initial assessment of patients and families to identify home‑care needs and admission problems.
• Outline and coordinate patient/family referrals and admissions.
• Use assessment data to develop and initiate personalized care plans.
• Articulate the hospice philosophy, goals, and objectives to patients and families.
• Communicate and collaborate with the clinical team to align care plans.
• Market hospice services to acute hospitals in the area.

Required profile

• Associate’s Degree in Nursing (required); Bachelor’s Degree preferred.
• 2‑4 years of hospice nursing experience.
• Current Registered Nurse license in the state of operation.
