About the role

James River Home Health is seeking a compassionate Home Health Admissions RN to guide patients and families through the transition to home care. The role involves initial assessments, eligibility determination, and coordination with the care team to ensure a smooth start of services.

Key responsibilities

• Conduct thorough initial assessments and determine patient eligibility for home health services.
• Explain services, answer questions, and guide patients and families through the admission process.
• Collaborate with physicians and the interdisciplinary team to gather medical information and ensure timely start of care.
• Maintain accurate documentation and communication to set the tone for high‑quality, patient‑centered care.

Required profile

• Registered Nurse with a current license to practice in the Commonwealth of Virginia.
• At least one year of home health experience preferred (not required).
• Proficiency in using electronic medical record (EMR) systems.

Required skills

• Electronic Medical Record (EMR) system usage.
